New Customers Information

We are thrilled to welcome you and your furry family members to the Tailored Animal Care family. To make your first mobile visit as smooth, efficient, and stress-free as possible, we ask that you complete our new client paperwork prior to our arrival.

Completing this form in advance allows Dr. Loomis to review your pet’s medical history and prepare any specific medication or equipment required for their visit.

• Complete the Online Form: Fill out the Google Form embedded below (or click the button to open it in a new window).

• Request Previous Records: If you have records from a previous veterinary clinic, please email them to Tailored.animal.care.vet@gmail.com or have a printed copy ready.

• Secure Your Pets: Remember to place cats in a secure bathroom 30 minutes before we arrive, and keep dogs inside.

• Write Down Questions: Jot down any questions about behavior, nutrition, or mobility that you want to review during your one-on-one time with Dr. Loomis.
